Ambulatory surgery centers run on tight schedules. One day can hold dozens of cases, and each case carries its own pre-op checklist, consent forms, anesthesia record, implant log, and discharge note. When that record lives on paper or inside a system built for hospitals, staff spend hours chasing signatures, retyping patient details, and cleaning up charts before a claim can go out.

An electronic medical record built for the ASC setting changes that routine. It follows the case from the first call to the final payment, lets several team members chart at once, and pulls the numbers a center needs for quality reporting.

Still, the market holds dozens of vendors, and no single product fits every center. A GI center has different needs than a multi-specialty facility, and an eye center needs cataract templates, not spine notes. The seven providers below cover a range of specialties and sizes, and each entry explains why the software earns its spot, so you can trim your demo list before the sales calls begin.

1. HST Pathways

Many surgery centers comparing cloud charting tools end up looking at HST Pathways, a Nashville company that builds software only for ASCs. The company reports that more than 1,800 surgery centers and 75,000 users work on its platform, with 50 million cases documented and a customer retention rate of 97%. KLAS Research, which surveys healthcare software users, named the company the 2024 Best in KLAS winner in the Ambulatory Surgery Center Solutions category.

The charting product, HST eChart, was designed with input from nurses and physicians. A feature called LiveEdit lets multiple clinicians update the same chart at the same time, so the anesthesia provider and the circulating nurse do not wait on each other. Real-time completion percentages and automated task triggers flag missing documentation before the patient leaves. One client, UroPartners, reached 100% chart closure even as its case count grew by 71%.

What sets the company apart is how much surrounds the chart. HST Practice Management handles scheduling, supply tracking, billing, and collections, and it submits claims through a Waystar integration. HST Case Coordination gives surgeon offices a real-time view of the schedule through a mobile app, which replaces fax and email schedule sharing. HST Patient Engagement adds two-way texting and an online pre-assessment form that flows straight into the chart. HST Profit Forecast estimates the margin on a case before the center books it, and HST Analytics supplies prebuilt dashboards. The company lists more than 80 integrations with physician office and hospital systems, and supplies entered in eChart populate the billing side automatically.

2. Surgical Information Systems

Surgical Information Systems, based in Alpharetta, Georgia, has sold surgical software for close to three decades and reports that about 40% of freestanding U.S. ASCs run on its products. Its cloud platform, SIS Complete, launched in 2019 and bundles SIS Office for scheduling and billing, SIS Charts for nursing, anesthesia, and physician documentation, SIS Link for case requests from surgeon offices, and SIS Exchange for patient questionnaires.

SIS Charts supports concurrent charting and configurable workflows, and it received the Best in KLAS award for ASC solutions in 2022, 2023, and 2025. The company also added outsourced revenue cycle services after acquiring Surgical Notes, plus a compliance tool called ASC Logs, which suits centers that want one vendor for software and billing services.

3. Provation Apex

Provation, headquartered in Minneapolis and owned by Fortive since 2021, built its reputation in gastroenterology. Provation Apex, released in 2018, is a cloud-based procedure documentation platform that guides physicians through more than 3,000 procedures in GI, pulmonology, orthopedics, and pain management. As the physician records findings, the software generates the codes for every billable action, which reduces the back-and-forth with coders.

The trade-off is scope. Apex focuses on the procedure note rather than the full center operation, though Provation's 2020 purchase of MD-Reports added practice management tools to its lineup. A GI or endoscopy center that cares most about note quality and clean coding should keep this one on the list.

4. ModMed ASC

ModMed ASC connects to the company's EMA electronic health record and practice management system, so a patient's history, medications, and scheduling details flow from the clinic into the surgery center chart without retyping. The company says practicing physicians helped build it.

Nurses, anesthesiologists, and physicians can chart at the same time, and pre-op, anesthesia, and post-op notes share common fields, so a medication entered once appears on the other notes in real time. The software carries built-in ASC Quality Reporting measures and reports designed to support Joint Commission, AAAHC, and CMS requirements. Centers tied to a physician group that already uses ModMed for dermatology, ophthalmology, orthopedics, pain management, or urology gain the most from this connection.

5. Nextech IntelleChartPRO

Nextech serves ophthalmology and plastic surgery practices, and it extended its IntelleChartPRO EHR into the ASC with an offering that folds its practice management tools into one surgical chart. The company says the product covers intake, the surgical chart, post-op care, and billing, and it estimates the system could remove 80% of paper use in a surgery center.

For eye surgery centers that perform hundreds of cataract and refractive cases a month, a chart that already understands ophthalmic workflows saves setup time. Larger groups use it too; US Eye, a Sarasota-based organization with 50 locations, selected IntelleChartPRO for records management. The product suits a dedicated eye ASC far better than a multi-specialty center.

6. Compulink ASC Advantage

Compulink Healthcare Solutions, based in Newbury Park, California, offers ASC Advantage, a single-database system that combines EHR, case scheduling, inventory, registration, reporting, and billing. Its One-Tab EHR puts the surgical team's charting on one screen, and the PracticeWatch feature automates parts of billing.

The patient engagement piece covers online registration, pre-op and post-op questionnaires, electronic consent signing, and text reminders. End users can adjust workflows themselves. One administrator at LoneStar Ambulatory Surgery Center in Texas reported cutting front desk chart prep from two full-time staff to one after the switch.

7. Picis Experior Suite

Picis Clinical Solutions of Peabody, Massachusetts, spent decades building perioperative and anesthesia software for hospitals, and its Experior Suite brings that background to the ASC. The platform combines EHR, practice management, and perioperative tools, and it is modular, so a center can start small and add applications later.

Available modules include scheduling and registration, case costing, implant tracking, preference cards, credentialing, patient accounting, clearinghouse services, anesthesia billing, and a patient portal. Picis also emphasizes device integration, which lets anesthesia records pull data from monitors. A center that already runs Picis in a partner hospital may find the shared platform easier to manage.

Choosing the Right Fit

The right EMR depends on what a center performs, how many locations it runs, and which systems its surgeons already use in their offices. A GI center, an eye center, and a multi-specialty facility will each rank these seven differently. Before any demo, list the three or four workflow problems that cost the most staff time today, then ask each vendor to show how its product solves those exact problems. Software that fixes real bottlenecks pays for itself far sooner than software that simply checks the most boxes.
